Are you passionate about making a difference in the lives of young people? We have an exciting opportunity for two full-time Social Workers to join our dedicated Adult Social Care Transitions Team.
We are a committed team focused on supporting young people aged 17-25 as they transition from Children's Services to Adult Social Care. Our mission is to empower young people to navigate this critical phase with confidence and independence.
The key aspects of this role are
- Empowering Young People: Apply a strength-based approach to help young individuals and their families through this transition.
- Collaboration: Work closely with professionals from Children’s Social Care, SEND, and Housing to ensure a smooth and positive transition.
- Assessments: Complete Care Act assessments and undertake prevention and enablement tasks, direct work, and reviews of support plans.
- Mental Capacity Assessments: Undertake mental capacity assessments, participate in Best Interest meetings, and complete Cop DOLS applications and renewals.
- Safeguarding: Respond to safeguarding concerns, implement safeguarding plans and undertake Section 42 Safeguarding Enquiries, applying Making Safeguarding Principals to all aspects of safeguarding work.
- Meetings & Reviews: Participate in IRO meetings, Pathway Plans, and EHCP reviews.
- Caseload Management: Manage a diverse caseload and take part in weekly duty rotations.
- Funding Arrangements: Explore alternative funding options, including Continuing Healthcare and S117 aftercare.
- Co Production: Work collaboratively with individuals, their families, support networks, and partner agencies, applying a person-centred and strengths-based approach.
- Partnerships: Initiate and develop effective partnerships and joint working arrangements with other professionals and agencies to ensure comprehensive assessment of need and delivery of appropriate services.
- Flexible Working: Work flexibly to meet the needs of residents, including regular face-to-face visits.
- Documentation: Attend and present at safeguarding meetings and other forums, and accurately and proficiently write, Section 42 Enquiries, case notes, funding applications and assessments in a timely manner.
About You:
- You will hold a recognised Social Work qualification and be registered with Social Work England.
- You will have successfully completed your ASYE
- You will have a full driving licence and access to a vehicle for work purposes.

Career Progression Frameworks (CPF) have been introduced for all roles at Medway Council. These frameworks provide a mechanism for employees to take ownership of their development within their post and be rewarded accordingly. This means that the salary you will be offered will be at level A unless, based on an assessment against the CPF criteria, you demonstrate that you are meeting the criteria of level B or C. You can demonstrate knowledge, experience and skills to allow you to perform within this role. You will identify, through application, how you meet the elements detailed within the person specification, at either Level A, B or C. You will be supported to develop in your role through induction, probation and a supportive Performance Appraisal and Career Conversation. Through your induction you will be introduced to the Career Progression Framework (CPF), which will help you to identify your areas of strength and any development needs.
